What Is a Substance Abuse Professional (SAP)?
A Substance Abuse Professional (SAP) is a qualified specialist who evaluates a driver who failed or refused a DOT drug or alcohol test and manages their return-to-duty process — the mandatory step before that driver can legally drive again (49 CFR 40.281).
What does a SAP do?
The SAP evaluates the driver, recommends education and/or treatment, re-evaluates whether the driver has complied, and sets a follow-up testing plan — at least six unannounced tests in the first 12 months, which the SAP can extend up to five years.
When do you need a SAP?
After a positive test or refusal. Seeing a SAP is the only route back to safety-sensitive work — a driver can't retest their way back without completing the SAP-directed process, and a negative return-to-duty test.
Who qualifies as a SAP?
Only specific credentialed professionals under 49 CFR 40.281 — a licensed physician, psychologist, licensed clinical social worker, employee-assistance professional, or certified addiction counselor — who have completed DOT-specific qualification training and an exam and keep up continuing education.
